Transportation Liability Waiver

I give permission for my child/children to be transported in by staff members of the Redeemer Renaissance After-School Program or Lutheran Church of the Redeemer.

I, as parent/guardian of the below minor(s), hereby consent and agree to hold harmless, Lutheran Church of the Redeemer and Redeemer Renaissance, (together, “REDEEMER”) and any and all employees thereof, for any accident, injury or occurrence arising out of being transported.

I understand that my child may ride on the bus owned and operated by the Redeemer Renaissance After-School Program, driven by an individual with a Commercial Driver’s License.

I understand that child may occasionally ride on a bus owned and operated by a transportation company driven by an individual with a Commercial Driver’s License.
